I am a board-certified orthopaedic hand surgeon in Fayetteville, Arkansas. The foundation of my training is orthopaedic surgery, with fellowship specialized training in hand, wrist, elbow, and arm surgery. I am a partner with Ozark Orthopaedics in Fayetteville, along with several other orthopaedic surgeons.

Download, print them out, and fill them out at your convenience so you can save yourself the annoyance of filling them out in the office.
